Education and information about Advance Directives and resuscitation measures
Listening & support for you and your caregivers
Smooth transition between hospital and home or other health care facility
Support for spiritual and religious concerns
Ways to deal with depression, loss of control, anxiety, loneliness, fears, and other problems

We have many other resources available to you at St. Vincent Hospital to help keep you as independent and comfortable as possible.
We work closely with other outpatient Palliative Care Services in the community should you need this once you leave the hospital.
