Our organization style is a team approach with the Team Leader RN visiting the patient on the initial visit to determine patient and family needs.   This Team Leader is a resource person, holds team meetings and patient conferences, and updates the Plan of Care.   Under the Team Leader, there are 2-4 primary RNs who see the patient after the initial admission to provide further cares.  Our approach, as noted, is multidisciplinary.  Besides nursing, we have therapies, personal care workers, medical social worker. 

One RN says she enjoys Home Care because "you're thinking on your feet."  It allows for "independence and flexibility"

in determining your own schedule. The  patient  is followed by the same nurse throughout duration of care, so you're able to see how "caringinterventions impact patientimprovements."
